Please print and complete this repair order form and include it with your order. Or, simply include a note on a plain sheet of  paper with the information requested below. We do not return original packing. Do not include the manufacturer's original container if it is important to you.

IF YOU'RE SENDING MORE THAN ONE ITEM: Just fill out one Repair Order Form, but tape a note to each unit telling us the problem it's exhibiting, whether the problem is delayed and/or intermittent, and what you've done to try and fix it, if anything. If you don't remember what the problem is or no one told you, don't worry about it --- we still put everything through a complete diagnosis.
